This form is to be completed by each chapter president acknowledging that they have had a conversation with the chapter at a chapter meeting about the Northwestern University Fraternity & Sorority Life Standards of Excellence and that they have covered relevant policies (i.e. Northwestern University Student Handbook, Council Standards, chapter standards, inclusive of residential standards) governing student groups with the chapter.
