Wedding Ceremony Request Form

Wedding requests can be made up to a year in advance. This request form along with a $300 deposit is required to book a wedding date and time. A signed contract is required for all events.

Wedding rehearsals are normally scheduled the evening before the wedding ceremony after 5:00pm. One hour is allotted for the reservation.

Clergy/Officiant Information I am interested in having a Northwestern Chaplain officiate my ceremony I have my own officiant that will be officating my ceremony I haven't yet scheduled an officiant but will be finding someone who is NOT a Northwestern Chaplain

Are you planning on hiring an Outside Wedding Coordinator?

Our wedding fee includes our Chapel Wedding Coordinator to run the rehearsal and day-of details. You are welcome to hire an additional planner but they must follow the Chapel Wedding Coordinators direction.

Are you interested in hosting your reception in Parkes Hall? No Yes

The Parkes Hall Social Hall can seat up to 150 guests at 60" round tables. For traditional wedding receptions, it is recommended that no more than 120 guests be accommodated to allow for food/beverage serving space and a small dancing/mingling area. A variety of room setups can be arranged by renting tables/chairs from an external source, speak with your coordinator for more options.
